Sie identifizieren Schnittstellen zwischen den verschiedenen Professionen und den Anliegen der Patient:innen.\nIn Phase zwei (Entwickeln und Umsetzen) erarbeiten sie personenzentrierte Lösungen (beispielsweise für den Weg zum Supermarkt und das Einkaufen im Geschäft) und eruieren strukturelle\, regionale Anpassungen sowie die Finanzierbarkeit der Lösung. Sie reflektieren ihre Lösungen kritisch in der Kleingruppe und mit den Co-Lehrenden.
